Guidelines for Documentations

Documentation Guidelines

The Office of Disabilities & Access Services  [ODAS] recommends students who are seeking accommodation on the basis of disability to provide documentation from a qualified medical professional. That documentation should describe the disability/health condition and the functional impacts of the disability/health condition. Documentation needs to be recent and relevant to the post-secondary academic setting.

Documentation for Service

Documentation serve three specific functions:

To establish that the student can be considered a person with a disability, and therefore eligible for protection against discrimination on the basis of disability.

To inform the development of reasonable accommodation (auxiliary aids and services) designed to facilitate equal access to Leighton' environments on a case-by-case basis.

To supplement information from the student regarding the impact of the disability.

Scope of Documentation

A diagnosis of the disability/health condition.

A description of the current impact/limitations of the disability/health condition with specific focus on barriers to the educational and/or housing environment.

A discussion of whether the disability/health condition is permanent or temporary

If temporary, please provide information on the expected duration of the disability/health condition and timeline for re-evaluation.

Identification from a qualified healthcare provider, containing:

(a) Contact information
(b) License number
(c) Signature or electronic signature

Documentation is evaluated by ODAS advisors and used to, both, verify the disability and complement self-report of lived experience while determining appropriate academic accommodation.  All student cases are assessed individually.
